Certificate Issuance Notification System

System Outline
When a copy of the residence record is requested by and issued to a representative, a third party, or an attorney or other licensed agent, the registered person will be notified of the request and subsequent issuance of documents. Advanced registration into the system is required.
Eligibility:
- All registered residents of Tokushima City are eligible
- Previously registered residents of Tokushima City who have relocated but their residence records (Joyo) are still on record with Tokushima City are also eligible
- Registration in the notification system will be discontinued if residents relocate overseas.
How to Register:
- The application form to register for the certificate issuance notification system (
本人 通知 制度 事前 登録申請書 ) should be submitted to the Resident Division or any of the 14 branches of Tokushima City Hall. The application form is available at the above counters or can be downloaded from the Tokushima City website (in the search bar, enter “本人 通知 制度 (honnin tsuuchi seido)” in Japanese).
Requirements:
- Please bring the application for the certificate issuance notification system and the applicant’s ID (e.g. Driver’s license, residence card, Individual Number (My Number) card, etc.)
Notes:
- Information that will be provided through the system includes when/what/how many of any issued records were given and who requested the issuance of such records (i.e. classification of requesting person)
- Registration must be made by each individual household member who wishes to be included in the system.
- With proof of power of attorney, registration can be made by a representing proxy.
- Registration is valid for indefinite period of time.
- To change or discontinue your registration, please follow the procedure.
