When it comes to procedures for the aftermath of the loss of a loved one, depending on the relationship with the deceased, what is required for the process varies. Additionally, the procedures often extend across multiple departments, therefore becoming a large burden for the bereaved family. It is in the attempt to relieve some of this burden from the bereaved family that the “Condolences Counter” was established.
Note, those that can utilize this service are the bereaved family members of a loved one who was registered within Tokushima City.
●In the case you plan to utilize the Condolences Counter
It is required to make a reservation in advance.
●In the case you do not plan on using the Condolences Counter
In this case, you would follow procedure by dealing directly with the respective relevant departments. In this situation, advance reservation is unnecessary.
Steps of Service
1 Appointment (Reserve Beforehand)
Please reserve your appointment at least three days before you wish to be served, either by phone or by coming into the office (Community Life Consultation Division Counter) directly.
(Reservations may be fully booked far out in advance, so please contact the division about making your own as soon as possible)

Appointment Availability: Monday through Friday, 8:30 am - 5:00 pm (closed on weekends and national/New Years holiday)
Appointment Times: (1) 9:00am (2) 10:30am (3) 1:30pm (4) 3:00pm
2 Necessary Verification Items
Please prepare to bring any necessary items so that the Condolences Counter may verify the nature of your required service.
For any questions, please consult the appropriate section in our guidebook (attached).
3 Day of Appointment
Please bring all the necessary items you prepared with you on your day of appointment.

Points of Consideration When Using the Condolences Corner
Depending on the necessary procedures, you may not be able to complete all required items within one appointment, or may be guided to a separate department. In order for smooth proceedings on the day of appointment, you will be asked for any necessary information about both the deceased and yourself/those that have come to the appointment, after which the Condolences Counter will pass along the appropriate information to the according department(s).
